Inpatient Stays

If you or your loved one are currently in a hospital and are being told you will need to continue your rehabilitation journey at an inpatient rehabilitation hospital, Rehabilitation Hospital of Indiana (RHI) is here to help. View more information about what we treat.

How to Get Started

  1. Let your Discharge Planner or Case Manager know that you would like to go to RHI.
  2. Your Case Manager will send a referral for Acute Inpatient Rehabilitation to RHI. Refer a patient online here.
  3. One of RHI's Clinical Liaisons will work with you, your Case Manager, and your doctors to evaluate you and prepare the necessary paperwork for you to transition your care when you are medically stable.
  4. RHI will check your insurance benefits. View insurance accepted. If you qualify for admission, we will submit your paperwork for approval from your insurance company or Medicare.
  5. Your Clinical Liaison or Case Manager will communicate your coverage and any out of pocket expenses that will need to be paid before admission. View information on billing and financial assistance.
  6. You will then be given a date and time for transportation to RHI. This can be via family member, van, or ambulance if needed.
  7. Upon arrival at RHI, you will be taken to your room and a hospital Admissions person will come to your room to complete Admissions paperwork. On the day of admission please have family present to supply necessary documentation.

Documents to Bring on Admission Day:

  • Medical insurance card(s)
  • Driver’s license
  • Power of Attorney (POA) or legal guardian for minors or patients unable to sign consent forms for treatment on their own

Outpatient Therapy

RHI offers outpatient therapy services at all three of our Indianapolis locations. Each location offers a variety of different therapies.

How to Get Started

  1. Please send a referral order with patient contact and insurance information. Refer a patient online here. (Referral order can be sent to desired Outpatient Location)
  2. RHI will check your insurance benefits. View insurance accepted.
  3. Your Case Manager will communicate your coverage and any out of pocket expenses that will need to be paid. View information on billing and financial assistance.
  4. Once the referral is received and benefits checked, RHI will call to schedule an evaluation.
  5. On the day of treatment, please be at the location 15 minutes prior to your evaluation to complete paperwork and register. Being early allows our therapists to stay on time and be respectful of everyone's schedules and treatment times.
